 ***IMPORTANT Paystub Info*** Please check your paystub for your sick day and tax exemption status.  Information is located in a box in the middle of the sheet.

 Double Premiums For those employees not paid over the summer, double insurance premiums will be deducted from the next three paychecks.  The following is the payment breakdown:

4/30/12 payment for June’s insurance

5/15/12 payment for July’s insurance

5/30/12 payment for August’s insurance

6/15/12 will be for any insurance increase eff. 7/1/12 or will be zero if there is no increase.

If you terminate and do NOT wish to have insurance over the summer, please let DeeDee know and your premium will be refunded.
